Description: San Ysidro Health- National City (“SYH National City”) consists of a 44,600 square foot development consisting of a San Diego Program of All-Inclusive Care for the Elderly (“SD PACE”) center and a Federally Qualified Health Center (“FQHC”) Clinic along with onsite parking.

The SYH National City SD PACE Center will provide a range of onsite healthcare, wellness programs, supportive services, and activities to SD PACE member seniors at the facility. The SD PACE Center will serve seniors 55 years of age and older, and who are certified by the State of California as needing a level of care equal to nursing home services, but who are able to live safely and independently in the community with the help of SD PACE services. The goal of the SD PACE program is to allow seniors to age in place at home rather than to be prematurely placed in nursing homes or institutionalized care. The SD PACE center will serve 650 low-income seniors annually at full capacity.

The SYH National City FQHC Clinic will provide services including: medical, dental, behavioral health services, a pharmacy, diagnostic laboratory, social and support services, transportation services, eligibility assistance, and referrals to specialty care. The FQHC Clinic will serve an estimated 45,600 patient visits annually of which an estimated 98% will be low-income persons and/or residents of low-income communities.

SYH National City is part of an innovative public-private redevelopment initiative designed to create an age friendly community featuring the development of intergenerational homes along with health care services and community amenities in a transit- and pedestrian-oriented village.

Today, more than 55 years later, it provides innovative services to more than 160,000 people through a vast and integrated network of 50 clinic and program sites across San Diego County including: medical and dental clinics, behavioral health centers, HIV centers, WIC nutrition centers, mobile medical units, school-based health centers, chiropractic services, pediatric developmental clinic, teen clinic, senior health center and PACE services.
